Stassi dropped it, made a bad throw to 1B to complete the strikeout, Walsh made a weird attempt at it, then Fletcher threw the ball high to the plate as Abreu was coming home, Ohtani tried to catch the throw from Walsh, Abreu hit him, Ohtani landed weirdly.

It was a combined effort. Walsh should have caught that earlier throw from Ohtani that scored the first run. Stassi was horrible, but Walsh didn't help.

I think the issue is that Ohtani’s control was not there so it was a big risk leaving him in. It is cool to have a manager show some balls though. This time it didn’t work out

Stassi dropped a perfect belt high pitch, threw it in the dirt, Fletcher overthrows it by a mile causing Ohtani to jump and land in a vulnerable position with a 250LB dude sliding into him. 

If you were trying to draw up a disaster it couldn't be worse than that.
