It's the difference between the run expectancy of the inning before the player comes to bat and after the at bat is over. 

So if the run expectancy of an inning is 0.5 runs, and Mike Trout hits a lead off homerun, he gains 1 run to his RE24 since 1 run is put on the board, and the run expectancy for the inning is still 0.5 runs. If there is a runner on 3rd and 2 outs the run expectancy might be around 0.3. If the player makes the third out the batter loses 0.3 runs off his RE24.
