Warren Spahn. Juan Marichal. Nolan Ryan. Bob Gibson. Sandy Koufax. These guys could throw over 200 pitches in a game. Spahn and Marichal faced off this one time at the ballpark and both threw 16 innings and over 200 pitches. It speaks to incredible mechanics and athleticism that these pitchers were able to do so.

All the TJ surgeries these days have nothing to do with pitch count. I don't know what it is, but it's something else. Maybe all the breaking stuff and weird pitching mechanics.

Bad repeating mechanics due to attempting to over throw your pitches. 

I've seen more pitchers attempt to over throw and jerk their front side open that then drops the elbow and they have to push it to home plate. Too many Guru's not enough knowledge.

Which equals continued stress on the elbow and ligament. Which is why they call it at first a stress reaction. Then a sprain. Then a full tear. Example Ben Joyce. He was bouncing through his delivery and push off vs the Astros in the 9-6 game which caused him to leak front side then his elbow trailed and he was pushing it. Had some discomfort went against the Cubs 4 days later. Didn't get out of the M's game...
