Ok, I am guessing the 3 weeks is to check on measurable improvement.

I would think the wise move from here would be to shut him down for awhile and focus on getting his bat back in the lineup. Reevaluate pitching in the stretch run.

Yeah, I'm thinking that, even if he responds to the injections and his elbow continues to heal, he not pitch the rest of the season and becomes fulltime DH and then come back spring of next year ready to go.

So best case scenario he makes one start before the All-Star break.  Barria will not be going up and down anymore.  Richards, Heaney, Skaggs, Barria and Tropeano are our new starting 5 for the foreseeable future.  DH will be a lot of Luis, Albert and Marte.

Just make Ohtani the closer once he is totally ready to pitch again.    Then he can bat pretty much everyday, and have a huge impact in an area of TOTAL NEED.

AND, his arm isn't burnt out so soon.

Imagine his heat with movement + splitter closing games.  Mo Rivera potential?
