So Middleton went on the DL with elbow inflammation and then right when he returns, he needs Tommy John? Is that just a coincidence? Or did the medical staff not see something?

I was wondering the same thing, I'm guessing the inflammation was due to the damage and maybe they couldn't see it because it was so inflamed? Needed a specialist to identify the tear? Either way, just a sad thing for Keynan, hope he comes back and makes his money these guys count on their health for their careers and something like this can end it. Luckily, today TJ surgery is so common it's expected that they'll come back at 100%. Get better Keynan!

Given they make guys do strengths tests after games and the sort -- I doubt he was pitching injured.   The Angels under Eppler have seemingly always been uber-cautious with their pitchers.

They threw 88, didn't, barely ever threw sliders and most were done by age 32..

Check the CY Young votes for the 70s -- youll be amazed how many of them broke down early...    More guys break down now sure, there are also more guys throwing, more guys pushing their bodies to the limits and lots more media coverage than in the 70s.

There have always been elbows but seemingly there were more shoulders (rotator cuff) in the 70’s and 80’s....
